The skilled interpreter of MR of the knee must do more than simply identify the presence of a meniscal tear. Tears should be characterized with regard to morphology, size, location, and stability, all of which are important factors that may influence the choice of operative treatment. As orthopaedic surgeons increasingly consider meniscal repair, accurate pre-operative assessment with MR becomes more important, allowing proper planning on the part of both the surgeon and the patient.

Any other structure in the intercondylar fossa is abnormal and a displaced meniscal fragment is the most likely possibility.
  5. If you image a complete radial tear directly along the length of the tear you will see an absent or empty meniscus. These complete radial tears open up and give the impression that there is a part missing. However you will not find a displaced meniscal fragment. It is simply separation of the meniscal parts.

It is a misunderstanding that menisci should be homogeneously low in signal intensity on proton-density images. The meniscus does not have to be black. Only when the high signal unequivocally reaches the surface of the meniscus you can make the diagnosis of a tear. If there is doubt whether the high signal touches the surface, look at all the adjacent images. If there is still doubt, then do not diagnose a tear. If you have a questionmark in your head, say "meniscus is normal". (figure)

Surprisingly, the majority of meniscus tears are not associated with a sports injury or trauma.  The majority of tears are degenerative or “complex” and indicate that the meniscus simply wore out over time.  Meniscus tears come in many different shapes and sizes.  The various types of meniscus tears you will help determine whether or not you may need surgery. The type of meniscus tears will also determine whether or not the torn piece can be stitched or repaired, or whether the torn piece will simply need to be removed.  The type of meniscus tear you have will also determine whether or not you will likely have persistent pain on an ongoing basis or whether or not you can expect to feel better after a period of time. A recent paper (2019) demonstrated that surgery for a degenerative tear (partial meniscectomy) was associated with a 4-10 times higher risk for requiring a knee replacement.  Fat-suppressed proton density-weighted (4a) sagittal and (4b) coronal images reveal a horizontal tear of the posterior horn of the medial meniscus (arrows), extending to the tibial surface. Each knee joint has two crescent-shaped cartilage menisci. These lie on the inside (medial) and outside (lateral) edges of the top of the tibia or shin bone, also known as the tibial plateau. They act as shock absorbers for the knee as well as providing support and stability for the joint. Injuries to either meniscus can prevent normal use of the knee. Tears of the meniscus can occur from damage to the inside of the knee. The medial and lateral menisci (plural of meniscus) are semi-round, articular cartilage that act as shock absorbers and.. 2. When you look at a degenerative tear, the edges are simply frayed.  The tissue has been worn out from years and years of use. It’s like the front of a pair of blue jeans that you’ve worn for decades and the material just simply wore out. Complex degenerative tears usually do not create loose unstable pieces that require surgery.  Most people with degenerative complex tears will notice a significant improvement in their pain within a few weeks to months.
  3. There are different ways to classify meniscus tears.  We can classify them based on where the tear is.  We can also classify them by how they occurred.  Most tears actually occur without trauma or injury.  Last, we can classify tears according to their “stability”.  A stable meniscus tear is much less likely the require surgery than a very unstable tear.  Let’s run through these various classifications.

  5. (12a) A radial tear (arrow) truncates the central attachment/root of the posterior horn of the medial meniscus on this fat suppressed proton density-weighted coronal image.
  6. A meniscus tear is usually caused by twisting or turning quickly Radial tears may be repairable depending on where they are located. Horizontal and flap (oblique) tears are generally not repairable
  7. In the event of more severe meniscus tears such as a bucket handle tear, arthroscopic surgical procedures may be necessary to repair the lesion. The aim of surgery is to preserve as much of the meniscus cartilage as possible.

  1. On the other hand, degenerative meniscus tears which occur as a result of wear and tear are usually not associated with trauma. Traumatic meniscus tears come in all sorts of shapes and sizes. There are radial tears, which are short tears that extend from the inner margin of the meniscus and head towards the periphery or outer part of the meniscus.  There is a picture above of a radial tear.  There are also horizontal cleavage tears.  These are very common and literally cleave a big portion of the meniscus in half, so there are nearly two “c” shaped discs sitting on top of one another. Many horizontal cleavage tears can be treated without surgery.  In the past, we did not believe that a horizontal tear would heal if we tried to repair it.  Recent changes in our technique have shown that these horizontal cleavage tears might heal very well if they are repaired.  It is often best to avoid removal of any meniscus if possible.

  4. Meniscus tears are very prevalent.  Not all meniscus tears are similar.  There are different locations for the tear.  There are different patterns of meniscus tears.  Your age might help predict the type of tear that might be present in your knee.  Certain meniscus tears will calm down, stop hurting and do very well without surgery.  Other types of meniscus tears are able to be repaired (sutured) and thus will do better in the long run with surgery.  This post will review the different types of meniscus tears that exist and the options for treatment based on the type of tear you have.

The two most important criteria for meniscal tears are an abnormal shape of the meniscus and high signal intensity unequivocally contacting the surface on PD images. If you have a red zone tear, there is a chance arthroscopic surgery can go in and sew it up. If you have a white zone tear, it is most likely that your meniscus will be removed. All or some of it. The great majority of meniscus arthroscopic surgeries are to REMOVE meniscus tissue. Horizontal tears Horizontal tears divide the meniscus in a top and bottom part (pita bread). If horizontal tears go all the way from the apex to the outer margin of the meniscus, they may result in the formation of a meniscal cyst. The synovial fluid runs peripherally through the horizontal tear and accumulates within the meniscus and finally result in a cyst. The connection with the joint space is often lost, so they will not fill with contrast on MR-arthrography. The synovial fluid is absorbed and is replaced by a gelatinous substance. Meniscus tear treatment aims to reduce the pain and inflammation associated with a knee cartilage tear as well as restore strength, flexibility and function. Get information on plastic surgery procedures, view before and after photos, read about the latest trends and find board-certified plastic surgeons in your area These are far less common than degenerative tears and tend to occur after sports injuries or trauma.  The size, shape, and type of meniscus tear will determine whether or not the meniscus tear is “stable” or “unstable”.  That can guide us to determine whether or not your pain will improve or might persist.  For example, if a loose flap of the meniscus is created by an injury,  that flap is moving around within the knee and sometimes gives you the feeling that the knee is locked or catching. That means that you might have ongoing pain and mechanical symptoms with various activities.    Many people will not tolerate a large meniscus flap tear well and those patients with meniscus flap tears (or unstable meniscus tears) will usually (not always) go on to request an arthroscopy or a scope to try to either repair or remove that torn piece. Torn meniscus surgery is one of the most common knee surgeries. The type of surgery you require will depend on the size, shape and location of your meniscus injury (5a) A longitudinal tear of the posterior horn of the medial meniscus is illustrated. The tear results in a vertical signal abnormality on sagittal MR images. A flap tear of the meniscus is a combination of a longitudinal tear and a radial tear. They start on the free edge of the meniscus and will extend to the meniscal fibrocartilage Longitudinal tears follow the collagen bundles that parallel the contour of the meniscus. If a longitudinal tear has other components (horizontal or radial), then it is a complex tear violating the collagen bundles. This requires a higher energy trauma.

Two months later, the post-operative image (17b) reveals a repaired, normal appearing lateral meniscal body (arrow), with resolution of the previously seen displaced fragment.

MR is also able to assess the stability of meniscal tears,6 an important factor, as unstable tears require operative treatment for symptom relief. Of course, if a displaced meniscal fragment is identified, the tear is by definition unstable. When displacement is not evident on MR images, additional criteria that suggest tear instability include the presence of fluid signal intensity within the tear on T2-weighted images, a tear that is greater than 10mm in length, and tears with complex patterns (10a).Disclaimer:  Please read our disclaimer.  This is not medical advice and is for informational purposes only.  No doctor-patient relationship is formed.

Any reproduction of symptoms, pain or clicking is a positive response, suggesting a torn meniscus.(3a) A fat-suppressed proton density-weighted axial image through the knee joint demonstrates the C-shaped menisci. Not the symmetrical shape of the lateral meniscus (red outline) and the asymmetry of the medial meniscus (blue outline), where the posterior horn (asterisk) is significantly larger than the anterior horn.(6a) A radial tear of the body of the lateral meniscus also appears vertical on sagittal MR images (arrow), though in the case of radial tears, the lesion is oriented perpendicular to the c-shaped fibers of the meniscus.

Acute or traumatic meniscus tears occur as a result of an injury.  These tend to occur in younger patients and children.  A bucket handle meniscus tear or flap tear is a common type of traumatic meniscus tear.  These tears are often repairable.  That means that during the surgery, we will put the tear back into its normal position and stitch or suture it into place.  Taking the torn piece out should not be misconstrued as a “repair”.  7 Yao L, Stanczak J, Boutin RD. Presumptive subarticular stress reactions of the knee: MRI detection and association with meniscal tear patterns. Skeletal Radiology 2004; 33:260-264.The menisci are C-shaped fibrocartilages with concave upper surfaces and flat undersides that match their respective interfaces with the femoral condyles and tibial plateau. The lateral meniscus has a symmetrical C-shape, whereas the medial meniscus is more crescentic (3a), as the posterior horn of the medial meniscus is always larger than the anterior horn. The medial meniscus has a firmer capsular attachment than the lateral meniscus. This makes the medial meniscus less mobile and is one reason why the medial meniscus is more prone to injury.3 In adults, only the periphery of the meniscus remains vascularized. This region of the outer meniscus, sometimes referred to as the “red zone”, is thought to occupy approximately 15% of the peripheral meniscus.4 Tears that occur within the red zone of the meniscus are more likely to heal than those in the avascular, “white zone” of the meniscus. Tears that lie within or contact the red zone are also more likely to be amenable to meniscal repair. Currently, routine MR images do not reveal signal intensity differences between the red and white zones of the menisci.

Meniscus tears are extremely common knee injuries. With proper diagnosis, treatment and rehabilitation, patients often return to their pre-injury abilities. The meniscus is a rubbery, C-shaped.. With regard to tear morphology, the classic ideal candidate for meniscal repair is the peripheral longitudinal tear. Longitudinal tears do not disrupt the circumferential architecture of the meniscus, and thus repair of longitudinal tears leads to a meniscus with relatively normal biomechanical function. The tear should be eight millimeters or more in length, as shorter peripheral longitudinal tears are less likely to be symptomatic and may heal spontaneously. Radial tears, because they are oriented perpendicular to the c-shaped fibers of the meniscus, have a devastating effect upon meniscal function. Those that extend through the entire width of the meniscus are particularly harmful (16a,16b), and even if such tears appear stable following repair, they are unlikely to regain the ability to provide hoop stress to the meniscus.13 Radial tears have therefore classically been treated with partial meniscectomy, though evolving surgical techniques have led to successful reports of the repair of radial tears that communicate with the meniscal periphery.11 A recent report has even described the successful repair of radial tears of the medial meniscal root,14 utilizing a tibial tunnel through which sutures are placed in the avulsed meniscus, a technique similar to that used in patients undergoing meniscal transplantation.

Shown are a longitudinal tear that may develop into a bucket handle tear; a radial tear that may develop a parrot beak tear; and a horizontal tear that may develop a flap tear Displaced meniscal tears are by definition unstable, and should be repaired relatively quickly, as displaced meniscal fragments may fibrose and distort, making delayed repair difficult or impossible. The “best known” displaced tear that is amenable to repair is the bucket-handle tear. Although all bucket handle tears are repair candidates,16 the bucket handle tear is an example of when the more severe appearing tear is actually better for the patient. In other words, when the majority of the meniscus forms the “handle”, that requires tear formation near the meniscal periphery, resulting in a vascular site for operative repair. When appropriate, tears that appear to involve the periphery, or red zone of the meniscus, should be described as such (9a), thereby alerting the surgeon to the fact that the tear is more amenable to repair. The Tears attribute determines the rate of fire of the character and is represented by 3 arrows. This attribute is also known as the tear delay (the delay between successive shots, as opposed to the inverse 'tears per second'); a higher tears stat means a shorter delay

If an ACL tear is also present, meniscal repairs are more successful if the ACL is also repaired, likely due to the protection afforded by knee stability. Patients with ACL tears are also thought to be better candidates for meniscal repair because of the presence of serum-derived growth factors within the hemarthrosis that accompanies ACL tears.15 Tension. Shapes. There are 3 basic shapes of meniscal tears: longitudinal, horizontal and radial. Complex tears are a combination of these basic shapes.Abnormal shape can be the result of partial meniscectomy. So you need to know what procedure was performed during arthroscopy. Only when comparison is made with prior postoperative images, can you determine, if an abnormal shape is a new finding indicative of a new tear. The meniscus bucket handle tear is a tear that tears vertically in either zone. It does not cut across A horizontal tear (or flap tear) is the exact opposite of a longitudinal tear; it cuts across the meniscus.. Longitudinal tear (3)Flipped meniscus is a form of bucket handle tear. There is a capsular detachment or peripheral tear of the meniscus, usually the posterior horn. The posterior horn flips over onto the anterior horn.Partial thickness tears or full thickness tears: The other classification of the meniscus tear is related to the depth of the tear.

Horizontal cleavage, oblique, and complex meniscal tear patterns have traditionally been poor candidates for meniscal repair. When small, conservative therapy or simply rasping the meniscus may result in healing of these tear types. Larger, unstable tears of this type often cause mechanical symptoms, however, and therefore warrant operative treatment, usually via partial meniscectomy. Repair is sometimes attempted even with these tear types, particularly when the patient is young and substantial loss of meniscal tissue would lead to an unacceptable risk of future arthritis.11 Repair of these challenging tear types should only be attempted when the meniscal tissue is of good quality and a stable result is achievable. Adjunctive measures to promote vascularity and healing at the repair sites are also recommended. Bucket-Handle Tears: This is an extended form of a longitudinal tear. In this tear, a piece of the meniscus gets separated from the tibia and forms a flap that resembles a bucket handle
