With appropriate antibiotic treatment, mortality from scrub typhus is quite rare, and the recovery period is short and usually without complications. [3, 30] However, mortality is still approximately 15% in some areas as a consequence of missed or delayed diagnosis. [32] If severe complications such as acute respiratory distress syndrome (ARDS) arise, mortality may still be high. [33] The louse, flea, tick, or mite becomes a carrier of the bacteria when they feed on the blood of an infected person (epidemic typhus) or an infected rodent (any of the three typhus forms mentioned above). The highest rates are seen in the older adults and those who are malnourished. Children usually recover from typhus. People with underlying diseases (such as diabetes mellitus, alcoholism, or chronic renal disorders) also have a higher risk of mortality. Mortality for epidemic typhus that goes untreated can range from 10 to 60 percent, and mortality from untreated scrub typhus can range up to 30 percent.

Epidemic/louse-borne typhus

Scrub typhus or mite-borne typhus is an infectious disease caused by a specific type of bacteria called Orientia tsutsugamushi. It is spread and maintained by the larvae of mites. These larvae are called chiggers. They bite human skin and leave a characteristic mark called an "eschar" (scar). The disease presents nonspecifically, but may rapidly progress to dreaded complications such as lung and liver failure. "Typhus" means fever with stupor, or smoke. It was a dreaded disease in the pre-antibiotic era, causing the deaths of thousands of soldiers during World War II. It is said that during Napoleons retreat from Moscow, a greater number of soldiers were killed by typhus than by the Russian army. STEAM GROUP. Scrub Typhus S.Typhus. Request To Join. 2. Humans acquire scrub typhus when an infected chigger bites them while feeding and inoculates O tsutsugamushi pathogens. The bacteria multiply at the inoculation site, and a papule forms that ulcerates and becomes necrotic, evolving into an eschar, with regional lymphadenopathy that may progress to generalized lymphadenopathy within a few days. In experimental infection, humans developed an acute febrile illness within 8-10 days of the chigger bite. Bacteremia was present 1-3 days before onset of fever. [14]
